When filling out this form you will need:  Name of Course (NOT PERIOD), Student Username, Student Full Name (PEIMS), AND Year of Graduation.  MAKE SURE your students all have passed the Arp AUP or they will NOT be allowed on ClassServer.  Arp AUP Powerpoints and Quizes are located at:  http://www.arp.sprnet.org/default/district/TRAINING/training.htm#AUP

Make out a Complete Form for EACH COURSE (not one per Period).  If you teach 3 periods of World History then place ALL students in this Course on ONE form.
